Provide most recent Financial Statements prepared by someone who is independent of your dealership (someone other than your in-house accountant or controller).

On "signed and dated letter" company letterhead provide the following:

A. Legal Name of your Business:

B. Form of Business, (Corp,LLC, or Sole Proprietor):

C. Type of Business (HVAC,Window,Plumbing,etc)

D. Names of Owners of the Business (Individuals or Parent Corporation)

Submit with Package

The completed FHA Title I Dealer Enrollment Package must be submitted in its entirety. Use the provided worksheet as your checklist to ensure all enrollment documents from Section I and II are included. Any incomplete applications will delay the approval process.
